The assessment is the Imagination Index questionnaire and scoring process. You answer items that ask you to rate your mental imagery (e.g. vividness) for scenarios involving the six senses. Your responses are summed into raw scores per category, then converted to linear score and percentile score and used to assign status (aphantasia, phantasia, or hyperphantasia).
